Resumen

Los trastornos patelofemorales constituyen entidades multifactoriales, y el tratamiento quirúrgico exige detectar la extensión anatómica y biomecánica que origina los síntomas. El objetivo de del estudio es realizar una revisión narrativa sobre la evidencia científica publicada entre 2020 y 2026 sobre la osteotomía de anteromedialización de Fulkerson, analizando la justificación de la indicación de la intervención, planificación preoperatoria, principios técnicos forma de fijación, resultados clínicos y complicaciones. Se realizó una búsqueda estructurada en PubMed, Scopus y Google Scholar, siguiendo como guía los principios que marcan PRISMA 2020. Se encontraron inicialmente 406 registros, y después del proceso de selección, cribado y evaluación de la elegibilidad se incluyeron 18 estudios en una síntesis narrativa. La evidencia recogida mostró que la selección del procedimiento debe basarse en la alteración predominante, incluyendo aspectos como lateralización de la tuberosidad, maltracking, patela alta, enfermedad condral, estabilidad medial y deformidades rotacionales. Los estudios evidenciaron resultados clínicos y funcionales favorables, principalmente cuando la osteotomía se dirige a una alteración anatómica que admitía una corrección, mientras que la inclusión sistemática de la osteotomía a la reconstrucción del ligamento patelofemoral medial no mostró ventajas funcionales consensuadas. Las complicaciones relatadas fueron heterogéneas y la retirada del material de fijación fue una causa importante de reintervención. Como conclusión, la osteotomía de Fulkerson supone una opción útil en pacientes seleccionados, cuya efectividad depende de la indicación y planificación personalizada
